Electronic Discharge Letters
ExtraMed Discharge Manager e-Letters is a simple and effective IT system for generating electronic discharge summaries to allow efficient hospital discharge.Delivering an accurate, comprehensive discharge letter to the GP presents a challenge to acute NHS Trusts as this process is still largely paper based. It is estimated that up to 40% of discharge letters currently sent to GP's are late or contain significant errors. This has a significant impact on post-discharge care and can affect patient outcomes.
Using ExtraMed Letters Doctors are assisted to include all the information required to meet PCT service level requirements. Information can be collated from the hospital PAS system and pharmacy systems.
Discharge letters include details of medication on admission and discharge. Users can also create and use ExtraMed Snippets to add hospital standard chunks of text – for example advice on driving, saving the need to constantly rekey these.
The system improves efficiency and helps doctors to meet the targets for delivering discharge summaries to GPs, providing a clear and concise summary of work. Doctors will be prompted to collect all the information they require for the GP discharge letters during the inpatient stay. On discharge they can generate the letters themselves or dictate the letter for typing at a later date, with all of the required information already available in the letter.
Sites can streamline the process by using ExtraMed Beds or any electronic Bed Management Information System or Discharge Management System.
Discharge Manager e-Letters can custom formats for each Specialty from one installation.
Generated letters can be securely distributed electronically or posted to the GP.
